Job Description
Hospice RN Case Managers are responsible for providing skilled nursing care while developing, coordinating and directing all aspects of the interdisciplinary care and services provided to hospice patients and their families.

Responsibilities:

Admits patients to service, assesses the patient's and family's physical, psychosocial, environmental, safety and developmental needs. Develops an individualized plan of care based on the assessment.
Obtains informed consent from patients and families and assists in completing forms required for hospice admission per regulations.
Provides patients and their families with guidance regarding health care decision-making and options for making advance directives in coordination with the social worker.
Assesses all aspects of the patient's pain and other symptoms and develops an individualized symptom management plan. Anticipates, prevents and treats undesirable symptoms or secondary symptoms including use of non-pharmacologic measures.
Supports, instructs and educates the patient, family, caregivers, and facility staff.
Documents nursing assessments, problems, appropriate goals, care interventions, patient and family outcomes, and evaluations of these in the electronic record.
Accesses electronic device during patient visit and documents during or immediately after the visit or per regulation.
Consults, collaborates, and coordinates with the IDG and others involved in the patient's care all patient and family services based on the patient's and family's preferences.
Attends hospice interdisciplinary group (IDG) meetings, educational in-service trainings, and other agency meetings as requested by manager.




Maintains the dignity of the dying patient and supports the patient's and family's unique spiritual and cultural beliefs.
Provides holistic, family-centered care across treatment settings to improve the quality of life.
Knowledgably discusses and initiates non-pharmacologic interventions as appropriate to promote comfort, improve quality of life, and help manage emotional, spiritual, and physical distress.
Participates in the hospice program's quality assessment performance improvement program and in-service education related to areas of practice as appropriate.
Meets productivity standards as established by the manager.

Prints out and files visit documentation in facility (RCFE or SNF) binder or chart per facility preference at the time of visit.
Initiates and participates in facility patient care conferences, in coordination with other IDG members, to discuss and clarify hospice care goals and roles of team members.
Maintains communication with patient, family and facility staff regarding all aspects of patient care.
Maintains and updates level of knowledge and skills required to provide quality hospice nursing care.
Creates, documents, and maintains the medication list. Evaluates medications for side effects or interactions and communicates these. Effectively instructs patient, family, and/or staff use of medications and reconciles medication records.
Effectively communicates with and uses the hospice prescription plan, formulary/preferred drug list, and general hospice medication coverage philosophy.
Adheres to all agency policies and procedures, including updates, as communicated in written, spoken, and electronic forms.
Promotes a culture of service when representing company by at all times treating internal and external contacts with respect, remaining professional, communicating clearly, preventing conflict, following through on commitments, and actively listening. Provides supervisory visits for the hospice aide as directed per regulation.
Demonstrates all skills of a Registered Nurse, including those skills required by the agency through competency testing.
Provides complete, current visit coordination information for each patient in the caseload.

Ensures that plan of care changes are documented in the electronic record as indicated.

Completes required discharge procedures and required electronic documentation.

Participates regularly in Interdisciplinary Group (IDG) or other patient care conferences in person and via other forms of communication.

Ensures supervisory visits regarding the hospice or home health aide are conducted according to regulatory and agency policy requirements.


Qualifications:
Current California RN - Registered Nurse License
Graduate of an NLN accredited nursing school
Minimum one year of Hospice nursing experience in the last three years
Current CPR certificate
